IT WAS REPORTED THAT DURING A LAPAROSCOPIC PROCEDURE INVOLVING THE DEVICE, THE SHEATH OF THE TROCAR RIPPED, AND A PIECE FELL OFF INTO THE PATIENT. TO RESOLVE THE ISSUE, THE BROKEN PIECE OF THE SHEATH WAS RETRIEVED FROM THE PATIENT'S CAVITY. THERE WAS NO PATIENT INJURY.
