The most frequently reported specific device problem is “Premature Discharge of Battery” (29 reports).
4 FDA recall records name at least one of these models by its device identifier, 2 still open.
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. InjuryAug 6, 2026Adverse Event Without Identified Device or Use Problem
It was reported that the patient experienced an infection. The Cardiac Resynchronization Therapy Defibrillator (CRT-D) system was explanted. No further patient complications have been reported as a result of this event.
Patient problems: Unspecified Infection
MAUDE report 50162064 ↗InjuryJul 13, 2026Adverse Event Without Identified Device or Use Problem
IT WAS REPORTED THAT THE PATIENT EXPERIENCED EROSION AND POSSIBLE INFECTION. IT WAS ALSO REPORTED THAT DURING RIGHT ATRIAL (RA) LEAD EXPLANT, A STYLET WAS INTRODUCED INTO THE RA LEAD LUMEN, THE HELIX WAS RETRACTED, AND THE LEAD WAS WITHDRAWN FROM THE RIGHT ATRIUM. HOWEVER, THE LEAD COULD NOT BE REMOVED PAST THE LEFT SUBCLAVIAN VEIN DESPITE MULTIPLE ATTEMPTS. FLUOROSCOPY SHOWED THE RA LEAD HAD BEEN DAMAGED DUE TO MANUAL STRESS DURING REMOVAL ATTEMPTS. AFTER THE STYLET WAS WITHDRAWN, IT COULD NOT BE RE-ADVANCED FULLY BECAUSE OF THE DAMAGED SEGMENT REMAINING IN THE SUBCLAVIAN VEIN. MULTIPLE SNARE ATTEMPTS VIA THE RIGHT FEMORAL VEIN WERE UNSUCCESSFUL, SO THE EXPOSED PORTION OF THE RA LEAD WAS CUT AND REMOVED, WHILE THE DAMAGED REMAINING SEGMENT WAS LEFT EMBEDDED IN FIBROUS TISSUE WITHIN THE LEFT SUBCLAVIAN VEIN AND REMAINS IN THE PATIENT. THE CARDIAC RESYNCHRONIZATION THERAPY DEFIBRILLATOR (CRT-D) SYSTEM WAS EXPLANTED. NO FURTHER PATIENT COMPLICATIONS HAVE BEEN REPORTED AS A RESULT OF THIS EVENT.
Patient problems: Erosion; Unspecified Infection
MAUDE report 25793593 ↗InjuryMay 12, 2026Adverse Event Without Identified Device or Use Problem
IT WAS REPORTED THAT THE CARDIAC RESYNCHRONIZATION THERAPY DEFIBRILLATOR (CRT-D) SYSTEM WAS EXPLANTED DUE TO VEGETATION IDENTIFIED ON THE RIGHT VENTRICULAR (RV) LEAD. NO FURTHER PATIENT COMPLICATIONS HAVE BEEN REPORTED AS A RESULT OF THIS EVENT.
Patient problems: Medical device site infection
MAUDE report 25155822 ↗InjuryApr 1, 2026Premature Discharge of Battery
IT WAS REPORTED THAT THE LEFT VENTRICULAR (LV) LEAD WAS MALPOSITIONED AND NONFUNCTIONAL. IN ADDITION, THE CARDIAC RESYNCHRONIZATION THERAPY DEFIBRILLATOR (CRT-D) EXHIBITED PREMATURE BATTERY DEPLETION. DURING THE REVISION PROCEDURE; IT WAS DETERMINED TO REPLACE THE LV LEAD WITH AN EPICARDIAL LEAD. DURING ATTEMPTED EXTRACTION; SIGNIFICANT SCAR TISSUE WAS NOTED ON THE LV LEAD. THE LV LEAD COULD NOT BE EXTRACTED. THE LV LEAD WAS CAPPED AND REPLACED. IN ADDITION, THE CRT-D HEADER WAS INCOMPATIBLE WITH THE EPICARDIAL LEAD. THE CRT-D WAS EXPLANTED AND REPLACED. NO PATIENT COMPLICATIONS HAVE BEEN REPORTED AS A RESULT OF THIS EVENT.
