ON 10/06/2023, HOLOGIC WAS MADE AWARE OF A CUSTOMER WHO QUESTIONED THE SAMPLE RESULTS FOR THEIR SARS/FLU A/B/RSV ASSAY EVALUATION FOR THEIR PANTHER FUSION INSTRUMENT (SN (B)(6) ). A CLINICAL PROFICIENCY SAMPLE WAS INITIALLY RUN ON (B)(6) 2023 USING REAGENT CARTRIDGE LOT 337024 AND RESULTED POSITIVE FOR SARS AND FLU B, WHICH WENT AGAINST THE EXPECTED RESULT OF A SINGLE SARS POSITIVE. THE CUSTOMER REPEATED THE SAMPLE ON (B)(6) 2023 ON THE SAME INSTRUMENT AND GOT ANOTHER DUAL POSITIVE RESULT FOR SARS AND FLU B. UPON FURTHER REVIEW OF THE INSTRUMENT LOGS, CUSTOMER NOTED THAT OUT OF 40 SAMPLES TESTED THAT WERE SARS POSITIVE, 31 OF THOSE SAMPLES WERE ALSO POSITIVE FOR FLU B. CUSTOMER QUESTIONED THE VALIDITY OF THESE SAMPLE RESULTS AND IF THE INSTRUMENT THERMOCYCLER MAY HAVE CONTRIBUTED TO THE SUSPECT RESULTS. THE CUSTOMER DID NOT INFORM HOLOGIC OF ANY RESULTS REPORTED TO PATIENTS, ANY CORRECTIONS OR AMENDMENTS TO THESE RESULTS, OR ANY TREATMENT PROVIDED. THERE WERE NO ASSOCIATED OR REPORTED ADVERSE EVENTS. AS PART OF EUA AGREEMENT, FDA REQUIRES ALL APTIMA SARS-COV-2 ASSAY QUESTIONING RESULTS AND FALSE RESULTS (CONFIRMED OR NOT) COMPLAINTS TO BE REPORTED AS MALFUNCTION MDR.

ON (B)(6) 2023, THE CUSTOMER REPORTED TO HOLOGIC THAT THEY SUSPECTED ONE SAMPLE WAS A FALSE POSITIVE FOR SARS-COV-2, FLU A, AND RSV. THE CUSTOMER WAS USING THE SARS-COV-2/FLU A/B/RSV ASSAY, REAGENT CARTRIDGE LOT 884490, ON THE PANTHER FUSION INSTRUMENT (SN (B)(6) ). THE SAMPLE IN QUESTION WAS INITIALLY TESTED ON (B)(6) 2023 AND RESULTED POSITIVE FOR SARS-COV-2, FLU A, AND RSV. CUSTOMER QUESTIONED THE SAMPLE RESULT SINCE ALL POSITIVE ANALYTES HAD SIMILAR CT VALUES. THE SAME SAMPLE TUBE WAS RETESTED ON A NON-HOLOGIC PLATFORM THE SAME DAY AND RESULTED NEGATIVE FOR SARS-COV-2, FLU A, AND RSV. ONCE THE RESULT ON THE NON-HOLOGIC PLATFORM WAS OBTAINED, THE CUSTOMER CHECKED THE CURVES FOR THIS SAMPLE AND SUSPECTED THAT IT WAS A FALSE TRIPLE POSITIVE. CUSTOMER THEN RETESTED THE SAME SAMPLE TUBE AND A NEW FUSION TUBE, BOTH TIMES RESULTING NEGATIVE FOR ALL FOUR ANALYTES. THE TRIPLE POSITIVE RESULT WAS REPORTED TO THE PATIENT, WHICH GOES AGAINST THE SARS-COV-2/FLU A/B/RSV ASSAY PACKAGE INSERT TO RETEST THE SAMPLE AND CONFIRM RESULTS SINCE TRIPLE INFECTIONS ARE RARE. THE CUSTOMER COULD NOT CONFIRM WHETHER THE PATIENT RECEIVED TREATMENT OR IF THE RESULT WAS AMENDED. THERE WERE NO ASSOCIATED/REPO

HOLOGIC BECAME AWARE OF A POTENTIAL FALSE POSITIVE HMPV RESULT SUBMITTED TO THE FDA (MDR REPORT KEY (B)(4). THE INCIDENT OCCURRED ON (B)(6) 2019, BUT HOLOGIC BECAME AWARE OF THE INCIDENT ON 01/05/2023. A PATIENT WAS TESTED FOR HUMAN METAPNEUMOVIRUS (HMPV) AND RHINOVIRUS (RV) USING THE PANTHER FUSION ADV/HMPV/RV ASSAY LOT: 247667. THE SAMPLE WAS RV-POSITIVE WITH A STRONG PCR SIGNAL AND HMPV-POSITIVE WITH A WEAK PCR SIGNAL. WHEN RETESTED, THE SAMPLE TEST RESULTS WERE STILL RV- AND HMPV-POSITIVE. THE FOLLOWING WEEK, TWO OTHER PATIENTS TEST POSITIVE FOR BOTH RV AND HMPV WITH THE SAME PCR SIGNAL RESULT, LEADING THE USER TO QUESTION THE INITIAL PATIENT¿S SAMPLE RESULT AS A POTENTIAL FALSE POSITIVE. THE INFORMATION REPORTED IN THE MDR IS INSUFFICIENT TO CHARACTERIZE ANY SAMPLE AS A CONFIRMED FALSE RESULT. HOLOGIC WAS NOT CONTACTED NOR MADE AWARE OF ANY RESULTS REPORTED, ANY AMENDMENTS TO RESULTS, OR ANY TREATMENT PROVIDED. USER CONTACTED THE VENDOR, WHO STATED THEY UNDERSTOOD THE CAUSE OF THIS ISSUE AND DID NOT ISSUE GUIDANCE TO CUSTOMERS. A PREVIOUS EVENT SEARCH PERFORMED ON (B)(6) 2023 FOUND NO SIMILAR COMPLAINTS ASSOCIATED WITH LN: 247667 SINCE ON (B)(6) 2019; THE ISSUE WITH ASSAY LOT:

CUSTOMER REPORTED THEY HAD DUAL POSITIVE SAMPLES (UNKNOWN NUMBER OF SAMPLES) FOR RV AND HMPV WHEN RUNNING THE ADV/HMPV/RV ASSAY ON THE PANTHER-FUSION. THESE DUAL POSITIVE SAMPLES HAD EARLY RV CYCLE TIMES ASSOCIATED WITH HIGH VIRAL CONTENT AND LATE HMPV CYCLE TIMES WITH A SLOW RAMP UP TO POSITIVITY. PER THE CUSTOMER'S PROCEDURE, THEY RETESTED THE DUAL POSITIVE RESULTS USING A NEW ALIQUOT OF VIRAL MEDIUM PLACED INTO A NEW STM TUBE. ONE SAMPLE THAT WAS ORIGINALLY DUAL RV AND HMPV POSITIVE THEN HMPV RETESTED AS NEGATIVE. HOLOGIC TECHNICAL SUPPORT (TS) REVIEWED THE CUSTOMER PANTHER LOGS. HOLOGIC DETERMINED THE CUSTOMER RESULTS SHOULD NOT BE COMPARED AS THEY ARE FROM DIFFERENT ALIQUOTS. THE RESULT DISCREPANCY WAS LIKELY TO BE DUE TO A LOW TARGET. TS INFORMED CUSTOMER THAT THERE IS A RISK OF HMPV OR ADV FALSE POSITIVE RESULT FOR SAMPLES CONTAINING A HIGH RHINOVIRUS VIRAL CONTENT. A RISK ASSESSMENT WAS COMPLETED, AND THE OVERALL RISKS ASSOCIATED WITH DUAL POSITIVE RESULTS IS THAT THE PROBABILITY OF INCORRECT RESULTS IS REMOTE AND THE CONSEQUENCES ARE NEITHER SEVERE NOR LIFE-THREATENING. ALTHOUGH THE POSSIBILITY OF DUAL POSITIVE RESULTS IS PART OF HOLOGIC'S LABELING, HOLOGIC UPDATE THE LABE
