We found 17 models of Z1 Hip System by Zimmer, Inc. in the FDA's device registry (GUDID). Together they are named in 25 adverse event reports in the FDA's MAUDE database, received between Feb 11, 2025 and Aug 27, 2026 — classified as 6 injuries, 19 malfunctions.
The most frequently reported specific device problem is “Positioning Failure” (8 reports).
1 FDA recall record name at least one of these models by its device identifier, 1 still open.
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. IT WAS REPORTED THAT DURING A RIGHT HIP PROCEDURE, THE FEMUR WAS BROACHED TO A SIZE 2 AND THE SIZE 2 STEM IMPLANT WENT RIGHT INTO THE FEMUR BY HAND WITHOUT IMPACTION. THE SURGEON DID NOT WANT TO BROACH UP BIGGER SO OPENED A SECOND STEM OF THE SAME SIZE. THE SECOND STEM SEATED APPROPRIATELY AND WAS IMPACTED WITH GREAT RESULT. THERE WAS NO KNOWN IMPACT OR CONSEQUENCES TO THE PATIENT. ATTEMPTS HAVE BEEN MADE AND NO FURTHER INFORMATION HAS BEEN PROVIDED.
