Summary

The FDA's MAUDE database contains 6 adverse event reports that list this exact device (DI 00802526480676), received between Nov 28, 2018 and Apr 16, 2026 — classified as 3 injuries, 3 malfunctions.

The most frequently reported specific device problem is “Premature Discharge of Battery” (2 reports).

14 other recalls cover devices cleared under the same FDA submission (P960040); they may or may not involve this specific model.

Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event, and counts can't be used to compare devices or estimate failure rates. How this data works · Data updated Oct 13, 2026.

  • MalfunctionApr 16, 2026Interrogation Problem

IT WAS REPORTED THAT THIS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR (ICD) HAD DIFFICULTIES TO INTERROGATE. THE HEALTH CARE PROFESSIONAL (HCP) CONSULTED FOR RECOMMENDATIONS TO INTERROGATE THIS DEVICE, TECHNICAL SERVICES (TS) REVIEWED THE DEVICE DATA, CONFIRMED THE DEVICE IS SUPPORTED BY THE PROGRAMMER AND RECOMMENDED TO HOVER THE WAND IN A CIRCULAR MOTION. THE HCP WAS REFERRED TO THE LOCAL FIELD REPRESENTATIVE. NO ADVERSE PATIENT EFFECTS WERE REPORTED. THIS ICD REMAINS IN SERVICE.

Patient problems: No Clinical Signs, Symptoms or Conditions

  • MalfunctionAug 14, 2025High impedance; High impedance

IT WAS REPORTED THAT A SHOCK IMPEDANCE MEASUREMENT OF GREATER THAN 200 OHMS IN TRIAD CONFIGURATION, WAS OBSERVED FOR THIS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR (ICD). IMPEDANCE TREND DATA SHOWED STABLE VALUES. THIS PATIENT WAS EVALUATED IN THE CLINIC IN WHICH IS WAS REVEALED THAT IMPEDANCE TESTING OCCURRED DURING PREMATURE VENTRICULAR CONTRACTION (PVC) ABLATION RESULTING IN THE OUT OF RANGE MEASUREMENT. TESTING CONFIRMED IMPEDANCES WERE BACK IN THE NORMAL RANGE. THIS ICD REMAINS IN SERVICE. NO ADVERSE PATIENT EFFECTS WERE REPORTED.

  • InjuryFeb 13, 2025Premature Discharge of Battery; Noise, Audible; Premature Discharge of Battery; Noise, Audible

IT WAS REPORTED THAT THIS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR (ICD) RECORDED A CODE 1003 INDICATIVE OF BATTERY VOLTAGE TOO LOW FOR THE PROJECTED REMAINING CAPACITY AND EMITTED A TONE PATTERN WITH 1.5 YEARS BATTERY REMAINING. THIS ICD WAS SUBMITTED FOR ENGINEERING ANALYSIS WHICH RESULTED THAT THIS DEVICE EXHIBITED A LOW VOLTAGE AND THE POWER CONSUMPTION WAS INCREASING IN GRADUAL FASHION. THIS APPEARED CONSISTENT WITH CAPACITOR DEGRADATION DUE TO HYDROGEN GAS CONTENT IN THE SEALED DEVICE ATMOSPHERE. DEVICE REPLACEMENT WAS RECOMMENDED. THIS ICD WAS EXPLANTED AND REPLACED. NO ADDITIONAL ADVERSE PATIENT EFFECTS WERE REPORTED.

BOSTON SCIENTIFIC RECEIVED INFORMATION THAT THERE WAS TWO UNTREATED EPISODES NOTED WITH THIS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR (ICD). MOREOVER, THERE WERE AN UNDERSENSED BEATS NOTED. BOSTON SCIENTIFIC TECHNICAL SERVICES (TS) THEN DISCUSSED ADJUSTING SENSITIVITY AND SUGGESTED MEASURING THE SIGNAL AMPLITUDE ON THE PROGRAMMER. TS ALSO DISCUSSED COULD CONSIDER GOING BACK TO NOMINAL DURATION SETTINGS. FURTHER, THE TS EXPLAINED THAT THE DEVICE WOULD NOT DIVERT TWICE IN A ROW. THE ICD REMAINS IN SERVICE. NO ADVERSE PATIENT EFFECTS WERE REPORTED.
