MAUDE report 50905816 ↗MalfunctionSep 14, 2026Signal Artifact/Noise; Over-Sensing; Low impedance
It was reported that this implantable cardioverter defibrillator (ICD) system exhibited oversensing of noisy signals for its right ventricular (RV) lead. Technical Services (TS) was consulted and reviewed stored data data. TS discussed stored data, with a decrease in impedance measurements for the RV lead. The impedance measurements were still within range. TS suspected a lead integrity issue based on the age of the RV lead. This device remains in service. No adverse patient effects were reported.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50755481 ↗InjuryJul 28, 2026High impedance
It was reported that implantable cardioverter defibrillator (ICD) exhibited high out-of-range (OOR) shock impedance measurements on the right ventricular (RV) lead. Technical Services (TS) recommended considering RV lead replacement if the average shock impedance exceeds 150 ohms. Additional information was received indicating that a Defibrillation Threshold Test (DFT) was performed. Shock impedance measurements were found to be within range. This ICD was subsequently explanted, and another ICD was implanted to complete this procedure. This ICD has not been returned at this time. No additional adverse patient effects were reported.
Patient problems: No Clinical Signs, Symptoms or Conditions

IT WAS REPORTED THAT THIS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR (ICD) RECORDED A CODE 1003, INDICATIVE THAT THE VOLTAGE WAS TOO LOW FOR PROJECTED REMAINING CAPACITY. TECHNICAL SERVICES (TS) REVIEWED THE AVAILABLE DEVICE DATA AND CONFIRMED THE BATTERY VOLTAGE HAD BEGUN DECREASING RAPIDLY, POTENTIALLY DUE TO AN INTERNAL BATTERY ISSUE. THE HEALTH CARE PROFESSIONAL (HCP) WAS ADVISED THAT PROMPT DEVICE REPLACEMENT WAS RECOMMENDED BECAUSE THE REMAINING BATTERY LONGEVITY COULD NOT BE ASSURED. NO ADVERSE PATIENT EFFECTS WERE REPORTED. THIS ICD REMAINS IN SERVICE.
