  2. Dec 12, 2024Open, Classified

Optical Sensors have been damaged due to physical interaction between the inlet and another device resulting in a Placement Signal Not Reliable (PSNR) alarm and loss of Central Venous (CV) Placement Signal, Pulmonary Artery (PA) Placement Signal & Pulmonary Artery Pulse Index (PAPi) metrics. Abiomed is reinforcing that there is a risk of potential interaction between the tip of guidewires, indwelling central venous lines or devices and inlet of the Impella pumps listed above during the insertion, manipulation, and removal of those devices. The interaction may result in optical sensor damage, temporary pump stop, or permanent pump stop.

CLINICAL NARRATIVE: AN IMPELLA RP WAS INSERTED VIA THE LEFT FEMORAL VEIN TO SUPPORT, ALONG WITH THE LEFT SIDED IMPELLA FOR A BIPELLA SUPPORT, FOR THE 57 YEAR OLD MALE PATIENT WHO HAD BEEN ADMITTED IN WITH CARDIOMYOPATHY. THE OTHER UNDERLYING MEDICAL HISTORY WAS NOT SHARED. THE PATIENT SUPPORT WAS NOT SUCCESSFULLY PLACED DUE TO NATIVE TORTUOUS ANATOMY. THE RP WAS EXPLANTED AND REPLACED BY AN RP FLEX PUMP WHICH WAS SUCCESSFULLY PLACED. THERE WAS NO HARM NOTED FROM ANY DELAY IN SUPPORT INITIATION. PATIENT HAS SURVIVED.

  • InjuryMar 24, 2026Adverse Event Without Identified Device or Use Problem

AN IMPELLA RP DEVICE WAS INSERTED INTO THE RIGHT FEMORAL VEIN IN A 61-YEAR-OLD FEMALE PATIENT WITH A PAST MEDICAL HISTORY OF A PRIOR CORONARY ARTERY DISEASE (CAD), DIABETES, AND RENAL INSUFFICIENCY, PRESENTING IN ACUTE MYOCARDIAL INFARCTION (AMI) AND CARDIOGENIC SHOCK (CGS), IN SCAI STAGE D SHOCK, ON MULTIPLE INOTROPES AND VASOPRESSORS, PRIOR TO INITIATION OF SUPPORT. DURING THE PROCEDURE, THERE WAS CONTINUOUS OOZING AROUND THE SHEATH DESPITE MATTRESS SUTURES X 3. THE FIRST SUTURE WAS PLACED PRIOR TO THE BLEEDING, THE SECOND AND THIRD SUTURES WERE PLACED TO STOP THE BLEEDING. MANUAL PRESSURE WAS APPLIED FOR OVER 30 MINUTES. THE OOZING RESOLVED BY THE END OF THE PROCEDURE. THE PHYSICIAN WAS NOT CONCERNED AND THERE WAS NO DROP IN THE HEMOGLOBIN. HEPARIN WAS GIVEN PRIOR TO IMPELLA PLACEMENT. THE IMPELLA RP WAS REMOVED THE SAME DAY, WITH A BRIDGE TO AN IMPELLA RP FLEX. THE POST-PROCEDURE OUTCOME IS SURVIVED AT EXPLANT. THE IMPELLA RP FUNCTIONED AT P-7 AT 3.7L/MIN AS INTENDED. BLEEDING IS A KNOWN RISK DUE TO THE IMPELLA'S ANTICOAGULATION AND PURGE REQUIREMENTS.

  • InjuryMar 9, 2026Failure to Advance

A 48-YEAR-OLD MALE WITH A HISTORY OF HEART FAILURE AND CARDIOGENIC SHOCK UNDERWENT IMPLANTATION OF AN IMPELLA 5.5 FOR LEFT-SIDED MECHANICAL CIRCULATORY SUPPORT. GIVEN THE EXTENT OF CARDIOGENIC SHOCK AND CONCERNS FOR RIGHT VENTRICULAR FAILURE, THE HEART TEAM PROCEEDED WITH AN ATTEMPT TO IMPLANT AN IMPELLA RP WITH SMARTASSIST (RPSA) VIA THE FEMORAL VEIN TO PROVIDE RIGHT-SIDED MECHANICAL CIRCULATORY SUPPORT. DURING THE PROCEDURE, THE TEAM WAS UNABLE TO ADVANCE THE IMPELLA RPSA CATHETER ACROSS THE RIGHT HEART AND INTO THE PULMONARY ARTERY. THE ATTEMPT WAS UNSUCCESSFUL DUE TO: SIGNIFICANT VESSEL TORTUOSITY, PREVENTING ADVANCEMENT TO APPROPRIATE PULMONARY ARTERY POSITION. GIVEN THE INABILITY TO SAFELY ADVANCE THE DEVICE, THE IMPELLA RP WAS REMOVED. THE HEART TEAM PLANNED TO PROCEED WITH PLACEMENT OF AN IMPELLA RP FLEX VIA AN ALTERNATIVE ACCESS SITE BETTER SUITED FOR THE PATIENT¿S ANATOMY REMAINED SUPPORTED BY IMPELLA 5.5 AFTER UNSUCCESSFUL RP IMPLANT. PLANS MADE TO PURSUE RP FLEX PLACEMENT FOR RIGHT-SIDED MECHANICAL CIRCULATORY SUPPORT VIA ALTERNATIVE ACCESS. NO HEMODYNAMIC INSTABILITY OR ADVERSE EVENTS REPORTED FROM THE UNSUCCESSFUL RPSA ATTEMPT.
