We found 34 models of Impella Heart Pump by Abiomed, Inc. in the FDA's device registry (GUDID). Together they are named in 30,421 adverse event reports in the FDA's MAUDE database, received between Nov 23, 2015 and Sep 30, 2026 — classified as 3,817 deaths, 15,140 injuries, 11,464 malfunctions.
The most frequently reported specific device problem is “Restricted Flow rate” (2,416 reports).
46 FDA recall records name at least one of these models by its device identifier, 42 still open.
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. A retrospective review identified that there are AIC units requiring specific hardware updates to address potential safety concerns. These hardware updates have been implemented through Abiomed s servicing process; therefore, AICs that have not received service are pending these updates. Issue 1: The proximity of the internal Video Graphics Array cable to the Digital Signal Processor chipset on the Impellatronic printed circuit assembly could potentially result in Electrostatic Discharge coupling into the Digital Signal Processor which may interrupt motor controls. Issue 2: There is a potential for the Compact Flash Memory Card dislodgement due to external applied forces, which could result in AIC startup failures and data logging issues. Issue 3: Improper routing of the fan wire within the AIC could lead to fan wire damage, which may result in console boot-up failure. Issue 4: Potential capacitor related issues on the Power Battery Manager, including the potential for the capacitors to cause pump stop, purge stop, and/or single fan fuse failures.

The complainant reported that a patient was implanted with an Impella CP via percutaneous left femoral artery for mechanical circulatory support. Placement signal not reliable was reported on the Automated Impella Controller (AIC) screen. The placement signal waveform was not visible on the AIC screen after the issue occurred. The pump's position was confirmed via echocardiogram. The procedure was successful with this device. The device was not removed due to the failure. The patient remains on support.

The complainant reported that a patient was implanted with an Impella 5.5 via right surgical axillary/subclavian artery for mechanical circulatory support. The patient¿s registered nurse (RN) called to say that the patient kept getting Impella in aorta and Impella position unknown alarms then resolving. They did an echocardiogram and the device position looked good per the team. The patient¿s RN stated no drops in flows, changes in blood pressure, or loss of pulsatility when the alarms occur. The medical doctor made the decision to disable placement signal alarms. The patient's outcome was stable. The patient remains on support.

MAUDE report 50948502 ↗MalfunctionSep 30, 2026Difficult to Remove
It was reported that the pump was originally pre-closed with per close two times. Upon insertion Abiomed representative instructed the physician that per close was not part of our IFU recommendations. The physician insisted on placing two anyway with acknowledgment of the risks. Upon successful wean and attempt to explant after removal of the repositioning sheath, there was resistance met at the motor housing portion of the pump removal. It was determined by the physician that the per close devices had been tightened around the catheter, and therefore the vein puncture site was not large enough to fit the motor out of the vein. The Abiomed representative was notified and they discussed that the safest option would more than likely be a cut down to remove the pump safely from the patient. Ultimately the physician was able to break the per close knot and remove the pump safely from the patient and removed the pump safely from the patient. No allegations were made against the device or the devices function with the doctor acknowledging that it was the result of the physician tightening the knots. Patient safe and stable at end of procedure. No patient harm was reported.
