IT WAS REPORTED THAT DURING A KNEE MENISCAL REPAIR, A CURVED MENISCAL REPAIR DEVICE BROKE INTRA-OPERATIVELY AND THE SUTURE BROKE. NO FRAGMENTS WERE RETAINED. HOWEVER, AFTER PRODUCT RETURN AND INVESTIGATION WAS DISCOVERED THAT THE DEVICE HAD PREMATURELY DEPLOYED. THE PATIENT HAD NO KNOWN IMPACT OR CONSEQUENCE. ATTEMPTS HAVE BEEN MADE AND NO FURTHER INFORMATION HAS BEEN PROVIDED.

MAUDE report 25665984 ↗MalfunctionMay 15, 2026Difficult or Delayed Activation
IT WAS REPORTED THAT OUTSIDE OF A PROCEDURE, THE JUGGERSTITCH WAS TESTED ON A SILICONE MENISCUS. WHEN ATTEMPTING TO INSERT THE FIRST IMPLANT, VERY HEAVY PRESSURE ON THE IMPLANT'S TRIGGER HAD TO BE USED AND PUSHING OUT THE FIRST IMPLANT WAS DIFFICULT. THE USER HEARD A CREAKING SOUND AS THE FIRST SHOT WAS ATTEMPTED AND EVENTUALLY THE FIRST ANCHOR CAME OUT, BUT WAS NOTED TO BE VERY THICK, FRAYED, AND SHORT. INSTALLING THE SECOND ANCHOR REQUIRED LESS EFFORT; IT LOOKED BETTER AND WAS NOTED TO BE LONGER THAN THE FIRST, THINNER, AND SMOOTHER. WHEN TIGHTENING THE ANCHOR, IT WAS NOTED TO BE RELUCTANT TO TAKE SHAPE, REQUIRING SOME EFFORT AND FELT AS THOUGH IT'S NOT TIGHTENING PROPERLY AND WAS TRYING TO COME OUT WITHOUT DEFORMING. ATTEMPTS HAVE BEEN MADE AND NO FURTHER INFORMATION HAS BEEN PROVIDED.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 25185163 ↗MalfunctionMay 8, 2026Difficult or Delayed Activation
IT WAS REPORTED THAT DURING A PROCEDURE, THE SURGEON TRIED TO INSERT THE ANCHOR AND THE ANCHOR WAS MISFIRED. ANOTHER JUGGER STITCH OF A DIFFERENT BATCH WAS USED TO COMPLETE THE PROCEDURE. NO PATIENT CONSEQUENCES WERE REPORTED AS A RESULT OF THE EVENT. ATTEMPTS HAVE BEEN MADE AND NO FURTHER INFORMATION HAS BEEN PROVIDED.
Patient problems: No Clinical Signs, Symptoms or Conditions

Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 24378228 ↗MalfunctionFeb 10, 2026Material Twisted/Bent
IT WAS REPORTED THAT DURING THE SURGERY, THE NEEDLE OF THE COMPLAINT PRODUCT'S DEVICE HAS ALREADY BEEN BENT WHEN THE SURGEON TRIED TO PENETRATE IT TO THE PATIENT'S MENISCUS. SO, THE SURGEON COULD NOT BE USED THE COMPLAINT PRODUCT FOR THE SURGERY. THE CORRECT SURGICAL TECHNIQUE WAS USED, AND AN ALTERNATE PRODUCT WAS USED TO COMPLETE THE PROCEDURE SUCCESSFULLY. NO COMPLICATIONS, INJURIES, OR SURGICAL INTERVENTIONS WERE REQUIRED, AND NO FOREIGN BODIES WERE RETAINED DUE TO THIS MALFUNCTION. IT WAS REPORTED THAT NO FURTHER INFORMATION IS AVAILABLE.

DURING INTRA-OPERATIVE USE, THE SURGEON ADVANCED THE BLACK DEPLOYMENT TRIGGER AND THE FIRST IMPLANT DID NOT DEPLOY. THE TRIGGER WAS RETURNED TO THE ORIGINAL POSITION AND ADVANCED AGAIN, AT WHICH POINT BOTH IMPLANTS DEPLOYED AT THE SAME TIME, PREVENTING COMPLETION OF THE INTENDED MATTRESS STITCH. DEVICE WAS LEFT IMPLANTED WITHOUT COMPLETING THE STITCH. ATTEMPTS HAVE BEEN MADE AND NO FURTHER INFORMATION HAS BEEN PROVIDED.

DAMAGE WAS THEN FOUND IN THE HANDLE AREA. TWO ANCHORS REMAINED IN THE KNEE AND THERE WAS A SURGICAL DELAY OF APPROXIMATELY TEN (10) MINUTES.
