Select CardioMEMS PA Sensors (Model CM2000) operate outside of the intended frequency range at higher elevations when readings are taken over approximately 2,000 ft. (610 meters) above sea level; operation outside of the intended radiofrequency has the potential to result in inaccurate readings or sensor signal acquisition difficulties.

  • InjurySep 23, 2026Separation Failure

It was reported that during CardioMEMS implant while attempting to deploy the sensor, the proximal loop of the sensor stayed attached to the device sheath. After unsuccessful troubleshooting, which included multiple breath holds and inserting a swan catheter next to the delivery catheter to try to ¿bump¿ the device off, the sensor needed to be snared in order to extract from the patient, and the implant procedure was aborted. An additional access site was used to remove the device. The physician confirmed that this issue caused a clinically significant delay as the procedure took longer than planned due to length of troubleshooting and the intervention needed to remove the device. A second attempt at implant is not scheduled at this time.

Patient problems: No Clinical Signs, Symptoms or Conditions

    MAUDE report 50869096 ↗
  • InjurySep 18, 2026Incorrect Measurement

A right heart catheterization was performed for unknown reasons and no recalibration was performed. Additional information was requested but has not been provided by the healthcare provider.

Patient problems: No Clinical Signs, Symptoms or Conditions

    MAUDE report 50816897 ↗
  • InjurySep 17, 2026Difficult or Delayed Separation

During the implant of the sensor, the sensor did not release from the distal end of the delivery catheter. The physician could not remove the sensor from the delivery catheter. The physician had to perform a second access site through the femoral vein to put the swan up. The balloon catheter was able to knock the sensor off of the catheter. The sensor was successfully implanted.
