Recalls naming this device

  1. Jun 3, 2025Open, Classified

A mechanical defect on a printed circuit board (PCB) as a result of PCB separating process. This defect might cause the ventilator to either fail at startup (no patient involvement) or during the use of the device enter ambient state, resulting in an interruption of ventilation.

  2. May 15, 2024Open, Classified

Ventilator may enter sensor fail mode, ventilation may not re-initiate, after patient is reconnected, which may cause hypoxia, if this sequence occurs: 1)User presses O2 enrichment, 2)User disconnects endotracheal tube for open suctioning, 3)Sensor error initiated, sensor fail mode occurs, 4)Patient is re-connected with sensor fail mode active, 5) Ventilation is not re-initiated by the ventilator.

  3. Mar 14, 2023Open, Classified

Software error causes, safety ventilation, in which ventilation continues in the "safety ventilation" mode with audible/visible alarm - patient inputs are not monitored, if the following coincide 1) A mode change to an adaptive mode (ASV, APVcmv, APVsimv, INTELLiVENT-ASV, (S)CMV+, SIMV+), and 2) The controller and/or humidifier is connected to the ventilator and is operational.

  5. Jun 27, 2022Open, Classified

Ventilator status indicator board can become loose, which could lead to water ingress (disinfectants) that may lead to technical fault alarms. Multiple technical faults in a short time may force ventilators into Safety Ventilation (blower runs constantly) or Ambient Sate (inspiratory channel/expiratory valves opened; patient breaths room air unassisted) with Panel connection lost message displayed

  • MalfunctionSep 30, 2026Incorrect, Inadequate or Imprecise Result or Readings

Customer states: On (b)(6).2026, during ventilation, the device displayed the error message ¿O¿ sensor failed¿ and stopped displaying the O¿ value. The FiO¿ was set to 30% when the message appeared. The patient¿s oxygen saturation subsequently decreased and stabilized at 91%. The FiO¿ was then increased to 100%, and the typical sound of oxygen flowing into the device during flushing was immediately heard. The device was replaced. No permanent patient harm was reported.

  • MalfunctionSep 28, 2026Gas/Air Leak

Hamilton Medical AG received the following description: Upon starting operation of the HAMILTON-C6, the device fails to reach the programmed tidal volume. The pre-operational check was repeated, and new consumables were installed; however, the issue persisted. The active humidification system was also adjusted, as excessive condensation was suspected of affecting the measurements, but this did not resolve the issue.Despite optimizing the consumable setup, repeating the pre-operational check, and adjusting the humidification, the issue remains unresolved. No harm to the patient was reported.

Patient problems: No Clinical Signs, Symptoms or Conditions

  • MalfunctionSep 24, 2026Gas/Air Leak

Hamilton Medical AG received the following event description: Upon starting operation of the HAMILTON-C6, the device fails to reach the programmed tidal volume. The pre-use check procedure was optimized, and the unit was restarted with new consumables, but the issue persisted. Active humidification settings were also adjusted¿considering that excessive condensation might be interfering with measurements¿but this did not resolve the problem either. Despite optimizing the setup of consumables, the pre-use check, and humidification settings, the issue remained unresolved. The replacement and verification of the complete patient circuit were reported as the medical intervention performed. No harm to the patient was reported.

  • InjurySep 24, 2026Improper Flow or Infusion

Hamilton Medical AG received the following description: "Suctioning was performed on the patient via the endotracheal tube. Suddenly, the patient's oxygen saturation dropped to 85%. The ventilator first issued a low minute volume warning, followed by a "patient circuit disconnected" alert. All connections were checked, but nothing was loose. The patient was not being ventilated. The flow sensor was replaced, but there was no change. A physician examined the airways using a fiberscope; mucus was found primarily within the endotracheal tube, but the airways themselves were clear. The ventilator was replaced, and the patient's condition stabilized. At the onset of the incident, the patient's EtCO2 waveform had flatlined, indicating that the patient was not actually being ventilated."No further patient consequences were reported.
