MalfunctionSep 21, 2026Difficult to Advance; Adverse Event Without Identified Device or Use Problem
Reportable based on device analysis completed on (b)(6)2026.It was reported that balloon advancing issue occurred. The 50-60% stenosed target lesion was located in the severely tortuous and mildly calcified arteriovenous fistula. A 6.0 x 200, 75cm Mustang balloon catheter was advanced for dilation. During the procedure, the balloon was fully deflated prior to advancement. However, significant resistance was encountered while advancing the catheter through the sheath. The procedure was completed with another of the same device. There were no patient complications as a result of this event, and the patient's condition post-procedure was stable. However, device analysis revealed a complete circumferential tear located at approximately 53mm distal of the proximal sleeve. A separation of the shaft was located at approximately at the same location of the complete circumferential tear, and the proximal markerband was found to be damaged.

REPORTABLE BASED ON DEVICE ANALYSIS COMPLETED ON 01APR2026. IT WAS REPORTED THAT DIFFICULTY ADVANCING OCCURRED. A 6.0 X 200, 75CM MUSTANG WAS SELECTED FOR USE. HOWEVER, DURING THE PROCEDURE, IT WAS NOTED THAT SIGNIFICANT RESISTANCE WAS FELT WHEN ADVANCING INTO THE NON-BOSTON SCIENTIFIC SHEATH. THE PROCEDURE WAS COMPLETED WITH ANOTHER OF THE SAME DEVICE. THERE WERE NO PATIENT COMPLICATIONS AS A RESULT OF THIS EVENT, AND THE PATIENT WAS STABLE POST-PROCEDURE. HOWEVER, DEVICE ANALYSIS REVEALED A BALLOON PINHOLE LOCATED APPROXIMATELY 4MM DISTAL OF THE DISTAL MARKERBAND.

MAUDE report 24923378 ↗MalfunctionJan 13, 2026Inflation Problem
REPORTABLE BASED ON DEVICE ANALYSIS COMPLETED ON 13JAN2026. IT WAS REPORTED THAT BALLOON FAILED TO INFLATE OCCURRED. A 6.0 X 200, 75CM MUSTANG BALLOON CATHETER WAS ADVANCED FOR VESSEL DILATATION. DURING INFLATION AT 23 ATMOSPHERES, THE BALLOON DID NOT EXPAND WELL IN AN AREA OF SEVERE STENOSIS. THE PHYSICIAN WANTED TO CHANGE HIGH PRESSURE BALLOON. THE PROCEDURE WAS COMPLETED USING A DIFFERENT DEVICE. THERE WERE NO PATIENT COMPLICATIONS AS A RESULT OF THIS EVENT. HOWEVER, RETURNED DEVICE REVEALED THE PRESENCE OF A BALLOON PINHOLE, WHICH LIKELY CONTRIBUTED TO THE INADEQUATE EXPANSION DURING THE PROCEDURE.

IT WAS REPORTED THAT A BALLOON RUPTURE AND DETACHMENT OCCURRED. THE TARGET LESION WAS A SEVERELY CALCIFIED, NON-TORTUOUS, TOTAL OCCLUSION LOCATED IN THE SUPERFICIAL FEMORAL ARTERY(SFA). A 6.0 X 200, 75CM MUSTANG BALLOON CATHETER WAS ADVANCED TO THE LESION AND INFLATED. ON THE FIRST INFLATION THE BALLOON RUPTURED CIRCUMFERENTIALLY AT A PRESSURE OF 10-12 ATM. WHILE PULLING ON THE BALLOON TO GET IT THROUGH THE SHEATH THE BALLOON CATHETER BROKE INTO TWO PIECES AT THE BALLOON SECTION. THEY ATTEMPTED TO REMOVE THE BROKEN PIECE BUT WERE NOT SUCCESSFUL. DUE TO THE FLOW THE BROKEN PIECE MOVED INTO THE KNEE. THE PATIENT WAS TRANSFERRED TO THE OPERATING ROOM, WHERE THE BROKEN BALLOON PIECE WAS REMOVED BY ATHERECTOMY IN THE POPLITEAL ARTERY. TWO NON-BSC STENTS WERE THEN PLACED IN THE SFA AND BLOOD FLOW WAS RESTORED. THAT NIGHT THE OUT FLOW VESSELS BELOW THE KNEE THROMBOSED AND THE PATIENT PRESENTED THE FOLLOWING DAY WITH A WHITE LEG DUE TO ACUTE ISCHEMIA. TWO DAYS LATER THE DECISION WAS MADE TO AMPUTATE THE LEG ABOVE THE KNEE. NO FURTHER PATIENT COMPLICATIONS WERE REPORTED.
