IT WAS REPORTED THAT DURING AN UNKNOWN SHOULDER PROCEDURE, A 6.5MM HEALX ADV SP PEEK ANCHOR ANCHOR DEVICE WAS USED. THE BONE QUALITY WAS NOT GREAT, HOWEVER, HELIX MEDIAL ROW WAS FINE AND A COMPETITORS DEVICE WAS ALSO FINE. THE 6.5 ANCHOR PULLED OUT. FOUR SUTURES GOING THROUGH THE ANCHOR. THE STAY STITCH WAS RELEASED BUT THE SHAFT WOULD NOT COME UNDONE FROM THE ANCHOR. ACCORDING TO THE DOCTOR "THE FRICTION BETWEEN THE INSERTER AND THE ANCHOR IS SO TIGHT THAT WHEN PULLING THE INSERTER OUT THE ANCHOR CAME WITH IT". SURGEON HAD TO LEAVE THE ANCHOR PROUD AND USE THE SUTURES COMING OFF OF THE ANCHOR WITH A LATERAL ROW SWIVEL LOCK TO HOLD THE SUTURES AND OUR 6.5 ANCHOR DOWN. THERE WAS NO DELAY IN THE PROCEDURE REPORTED. THERE WERE NO ADVERSE PATIENT CONSEQUENCES REPORTED. ALL AVAILABLE INFORMATION HAS BEEN DISCLOSED.
