It was reported that when the polyp was to be removed by the snare, it was found that the snare could not be retracted to remove the polyp. Therefore, use removed the device and replaced it for treatment.A section of the device did not remain inside the patient¿s body. MAUDE report 13501605 ↗InjuryAug 30, 2021Failure to Cut; Failure to Cut
DURING A POLYPECTOMY, THE PHYSICIAN USED TWO (2) ACUSNARE POLYPECTOMY SNARES. THEY CLOSED THE FIRST SNARE ONTO THE POLYP. THEY TRIED TO CUT WITH CAUTERY BUT THE CAUTERY WOULD NOT CUT THE POLYP. THEN, THE SNARE WOULD NOT OPEN FOR REMOVAL. THE SNARE WAS STUCK ON THE POLYP. THEY TRIED TO USE A SECOND SNARE TO CUT THE POLYP AND FREE THE FIRST SNARE, BUT IT HAD THE SAME PROBLEM WITH BOTH CAUTERY AND NOT REOPENING AND BECAME STUCK AS WELL. THE PATIENT WAS SENT TO SURGERY FOR A RIGHT HEMICOLECTOMY IN ORDER TO REMOVE THE SNARES AND THE POLYP. THE FOLLOWING WAS RECEIVED ON 09/03/2021- THE INTENTION OF THE ORIGINAL PROCEDURE TO REMOVE A LARGE COLON POLYP. SURGERY WAS AN UNEXPECTED COMPLICATION DUE TO THE MALFUNCTION. A SECTION OF THE DEVICE DID NOT REMAIN INSIDE THE PATIENT'S BODY. THE PATIENT WAS SENT TO SURGERY FOR A RIGHT HEMICOLECTOMY IN ORDER TO REMOVE THE SNARES AND THE POLYP DUE TO THIS OCCURRENCE.

DURING A COLONOSCOPY WITH POLYPECTOMY, THE PHYSICIAN USED AN ACUSNARE POLYPECTOMY SNARE. IN THE PROCESS OF A POLYP REMOVAL USING A LARGE SNARE BY THE PROVIDER, THE RN ASSISTANT WAS ABLE TO CLOSE THE SNARE TO THE "20" MARK BUT NO CAUTERY WAS NOTICED DESPITE CONFIRMATION OF OPERATIONAL GROUNDING PAD AND ELECTROSURGICAL GENERATOR. THE SNARE SHEATH BUCKLED WITH PRESSURE WHEN ENCLOSING THE POLYP TISSUE WITHIN THE SNARE. AN INADEQUATE CUT AND BURN OCCURRED AND THE PROVIDER WAS UNABLE TO LOOSEN THE SNARE OFF THE POLYP. THE PROVIDER THEN HAD TO MANUALLY CUT THE SNARE AT THE HANDLE, WITHDRAW THE SNARE AND SCOPE, AND GO BACK IN THE COLON TO RETRIEVE THE SNARE LOOP FROM THE PATIENT WITH FORCEPS. THEY COMPLETED THE PROCEDURE WITH A DIFFERENT SNARE. A SECTION OF THE DEVICE REMAINED TEMPORARILY INSIDE THE PATIENT¿S BODY, BUT WAS SUCCESSFULLY RETRIEVED WITH FORCEPS. ACCORDING TO THE INITIAL REPORTER, THE PATIENT DID NOT EXPERIENCE ANY ADVERSE EFFECTS DUE TO THIS OCCURRENCE.

THE PHYSICIAN THEN CHANGED TO ANOTHER MANUFACTURE'S SNARE TO COMPLETE THE PROCEDURE. HOT BIOPSY FORCEPS WERE USED TO CLEAN THE WOUND AND THE WOUND WAS CLOSED WITH A HARMONY CLAMP. A SECTION OF THE DEVICE DID NOT REMAIN INSIDE THE PATIENT¿S BODY. THE PATIENT DID NOT REQUIRE ANY ADDITIONAL PROCEDURES DUE TO THIS OCCURRENCE. DURING A COLONOSCOPY, THE PHYSICIAN USED A COOK ACUSNARE POLYPECTOMY SNARE. THE REGISTERED NURSE (RN) FROM THE FACILITY CALLED IN THE COMPLAINT DETAILING THAT THE COMPLAINT [DEVICE] WAS BEING USED TO ATTEMPT REMOVAL OF A VERY LARGE DENSE POLYP FOUND AT THE CECUM. THE SNARE WAS CONNECTED TO CAUTERY, INSERTED, AND PLACED AROUND THE POLYP. THE SNARE WOULD NOT FULLY CUT THE POLYP. AFTER THE THIRD ATTEMPT TO CUT THROUGH THE POLYP, IT WAS DECIDED TO REMOVE THE SNARE FROM THE PATIENT; HOWEVER, THE SNARE WOULD NOT RELEASE FROM THE POLYP. A GASTROENTEROLOGIST WAS CALLED AND HE WAS EVENTUALLY ABLE TO CUT A PORTION OF THE POLYP AND REMOVE THE SNARE FROM THE PATIENT. THE NURSE NOTED THE SNARE HAD A LOT OF TWISTING AND CRINKLING ABOUT 6-8 INCHES DOWN THE SHAFT. THE REPORTER CONFIRMED THE DEVICE WAS NOT TURNED OR TWISTED DURING THE PROCEDURE. THE USER CONCLUDED REMOVAL OF THIS POLYP, PULLED A DIFFERENT SNARE AND CONTINUED WITH REMOVAL OF OTHER POLYPS. THE POLYP WHICH ENCOUNTERED THE SNARE ISSUE WAS LEFT AS THE PATIENT IS RETURNING FOR A HEMICOLECTOMY UNRELATED TO THIS COMPLAINT EVENT. THERE WERE NOT ADVERSE EFFECTS NOTED, BUT THE PATIENT WAS UNDER PROLONGED ANESTHESIA. A SECTION OF THE DEVICE DI
