DURING AN ASCENDING COLON POLYP REMOVAL, THE PHYSICIAN USED A COOK ACUSNARE POLYPECTOMY SNARE. IT WAS REPORTED [THAT] THE WIRE AT THE DISTAL END OF THE SNARE WAS FOUND TO BE FRACTURED. THE DEVICE WAS IMMEDIATELY REPLACED WITH A NEW COOK SNARE AND THE POLYPECTOMY WAS COMPLETED. THERE WAS NO REPORTABLE INFORMATION AT THIS TIME. THE DEVICE WAS EVALUATED ON 10APR2026. IT WAS DETERMINED THAT THE SNARE TIP WAS FRAYED AND THE DEVICE WAS BARELY ABLE TO CUT/COAGULATE THE TISSUE [SUBJECT OF REPORT]. A SECTION OF THE DEVICE DID NOT REMAIN INSIDE THE PATIENT¿S BODY. THE PATIENT DID NOT REQUIRE ANY ADDITIONAL PROCEDURES DUE TO THIS OCCURRENCE. ACCORDING TO THE INITIAL REPORTER, THE PATIENT DID NOT EXPERIENCE ANY ADVERSE EFFECTS DUE TO THIS OCCURRENCE.

DURING AN ENTEROSCOPY INSPECTION IN COLON, THE PHYSICIAN USED A COOK ACUSNARE POLYPECTOMY SNARE SOFT. IT WAS REPORTED [THAT] USER OPENED THE PACKAGE AND ADVANCED THE DEVICE INTO INTESTINAL TRACT THROUGH ENDOSCOPE WORKING CHANNEL. WHEN READYING TO OPEN THE SNARE, IT WAS FOUND THAT THERE WAS GREAT RESISTANCE AND THE SUPPORTING SHEATH DEFORMED. USER RETRACTED THE DEVICE AND ENDOSCOPE, AND CHANGED TO ANOTHER DEVICE TO COMPLETE THE PROCEDURE. FURTHER INFORMATION DETERMINED THAT THE SNARE WAS DIFFICULT TO RETRACT. A SECTION OF THE DEVICE DID NOT REMAIN INSIDE THE PATIENT¿S BODY. THE PATIENT DID NOT REQUIRE ANY ADDITIONAL PROCEDURES DUE TO THIS OCCURRENCE. DURING A COLONOSCOPY, THE PHYSICIAN USED A COOK ACUSNARE POLYPECTOMY SNARE SOFT. IT WAS REPORTED [THAT] THEY PUT THE FIRST SNARE AROUND THE LARGE POLYP AND TRIED TO CUT THE POLYP OFF. THE SNARE WOULD NOT CUT THROUGH THE POLYP. THE SNARE THEN REMAINED STUCK ON THE POLYP. THEY COULD NOT GET THE SNARE OFF THE POLYP. SO, THEY CUT THE HANDLE OFF THE SNARE SO THEY COULD REMOVE THE SCOPE. THE SNARE LOOP WAS STILL STUCK IN THE PATIENT AND THE SCOPE WAS OUT. A SECTION OF THE DEVICE DID NOT REMAIN INSIDE THE PATIENT¿S BODY. THE SNARE LOOP LODGED ON POLYP WAS RETRIEVED WITH ANOTHER SNARE TO REMOVE POLYP DUE TO THIS OCCURRENCE. ACCORDING TO THE INITIAL REPORTER, THE PATIENT DID NOT EXPERIENCE ANY ADVERSE EFFECTS DUE TO THIS OCCURRENCE.

DURING A COLONOSCOPY WITH POLYPECTOMY, THE PHYSICIAN USED A COOK ACUSNARE POLYPECTOMY SNARE, SOFT. IT WAS REPORTED THAT THE POLYP STALK WAS VERY HARD, AND THEY WERE UNABLE TO CUT THRU THE STALK. SEVERAL SNARES WERE USED AS THEY STATED THEY THOUGHT THE SNARE WAS MELTING THE PLASTIC SHEATH AT THE BASE OF THE HANDLE BUT IT WAS DETERMINED THAT IT WAS NOT MELTING BUT KINKING DUE TO THE TENSION PLACED ON THE HANDLE AND THE WIRE NOT CUTTING THRU THE POLYP STALK. THEY WERE ABLE TO SUCCESSFULLY REMOVE THE POLYP WITH ANOTHER OF THE SAME DEVICE. A SECTION OF THE DEVICE DID NOT REMAIN INSIDE THE PATIENT¿S BODY.
