InjurySep 10, 2026Over-Sensing; Low impedance
It was reported that the patient presented for a follow-up in clinic. Upon interrogation, it was noted that the right atrial (RA) lead exhibited low pacing impedance, noise over-sensing and myopotential over-sensing. The RA lead was explanted and replaced. The patient remained in stable condition.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50707713 ↗MalfunctionSep 9, 2026Difficult to Remove
It was reported that the patient presented for a generator change procedure. During the procedure, it was noted that the right ventricular (RV) lead and right atrial (RA) lead could not be removed from the pacemaker header. The physician injected saline into the pacemaker header in an attempt to remove the leads. The RA lead was eventually removed; however, the insulation of the RV lead was damaged, and the RV lead could not be removed from the pacemaker header. The physician subsequently capped the RV lead. The existing RA lead and a new RV lead were connected to the new pacemaker, and the old pacemaker was explanted. The patient remained in stable condition throughout the procedure.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50685314 ↗InjurySep 8, 2026Break; Over-Sensing; Pacing Problem; Low impedance
It was reported that the patient presented to the hospital for a device changeout procedure. Prior to the procedure, it was noted that the right atrial (RA) lead exhibited oversensing noise resulting in inappropriate mode switch, low pacing impedance. During the procedure, it was noted that both the RA and right ventricular (RV) leads had compromised outer insulation and exposed wire. Both the RA and RV leads were repaired and remained implanted. The patient was in stable condition.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50659283 ↗InjuryAug 19, 2026Break; Failure to Capture; High impedance
It was reported that the atrial lead exhibited high pacing impedance and failed to capture when pacing at high outputs. During the procedure, the physician observed evidence of insulation damage on the atrial lead. When attempting to reconnect the existing atrial lead to the replacement pulse generator, high impedance was again noted on the atrial lead, and capture could not be achieved even at high pacing outputs. Subsequently, the atrial lead was capped on (b)(6) 2026 and replaced. The patient was stable throughout.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50315829 ↗MalfunctionAug 18, 2026Over-Sensing; Pacing Problem
It was reported that the patient presented for a follow up in clinic. It was note that the right atrial lead exhibited noise over-sensing which resulted in inappropriate mode switch. No intervention was performed and will continue to be monitored. The patient remained in stable condition.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50294194 ↗InjuryAug 5, 2026Over-Sensing; Failure to Sense
It was reported that the patient presented remotely via Merlin.net. Upon review of the transmission, far-field R-wave (Far-R) oversensing and atrial undersensing were identified. The patient subsequently presented to the emergency department for further evaluation. Upon device interrogation, ventricular non-capture was observed at maximum output in both unipolar and bipolar configurations. In addition, the atrial lead exhibited failure to sense. As a result, the pacemaker was explanted and replaced, and the atrial and right ventricular (RV) leads were capped and replaced on (b)(6) 2026. The patient was reported to be stable throughout the event and subsequent procedures.
