  • MalfunctionSep 19, 2025Failure to Form Staple; Failure to Form Staple

IT WAS REPORTED THAT DURING A THORACOSCOPIC LEFT UPPER LOBECTOMY, A MANUAL HANDLE WITH A RELOAD WAS USED FOR INTERLOBAR FISSURE DETACHMENT WHEN THE STAPLE BURST OUT, THE STAPLES LACKED PROPER B-SHAPED FORMATION AND THE STAPLE LINE WAS INCOMPLETE, RESULTING IN BLEEDING. AFTER REPLACING THE RELOAD, THE STAPLE LINE REMAINED INCOMPLETE AFTER FIRING AND ADDITIONAL BLEEDING OCCURRED IN THE MIDSECTION. THE SURGICAL TIME WAS EXTENDED BY 30 MINUTES OR MORE DUE TO THESE ISSUES. MANUAL SUTURING WAS PERFORMED UNDER LAPAROSCOPY TO RESOLVE THE INCOMPLETE STAPLE LINE AND BLEEDING.
