IT WAS REPORTED THAT THE ADULT BREATHING CIRCUIT INCLUDED IN THE CARDINAL GENERAL ANESTHESIA KIT APPEARS TO BE A NEWER VERSION THAN PREVIOUSLY SUPPLIED. DURING USE, THE CIRCUIT REPEATEDLY DISCONNECTED AT BOTH THE PATIENT END AND THE VENTILATOR END, INCLUDING DURING ACTIVE CASES AND DURING EXTUBATION, RAISING PATIENT SAFETY CONCERNS. NO HARM OR INJURY WAS REPORTED AS A RESULT OF THIS EVENT.
