AFTER THE 4TH RECURRENCE WITH SAME ERROR, SURGEON REMOVED DEVICE FROM TROCAR, ASKED SCRUB NURSE TO WIPE THE JAWS IN CASE THEY WERE NOT CLEAN, AND THEN SURGEON TRIED AGAIN. THE SAME ERROR OCCURRED.
