The most frequently reported specific device problem is “Material Rupture” (284 reports).
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. MAUDE report 50952612 ↗MalfunctionSep 29, 2026Material Rupture
On (b)(6) 2026, a patient underwent a percutaneous transluminal angioplasty (PTA) procedure under digital subtraction angiography (DSA) guidance using a Conquest balloon catheter. During the procedure, after the balloon catheter was advanced to the target lesion site, inflation was initiated using a pressure inflation device. However, the inflation device suddenly lost pressure and rapidly deflated. Multiple attempts were made to re-establish pressure; however, the system failed to maintain inflation. Upon evaluation, a cannula-like rupture of the balloon was identified at approximately 10 atm. There was no reported patient injury.

MAUDE report 50921287 ↗MalfunctionSep 28, 2026Material Rupture
A patient underwent an angioplasty procedure using the Conquest PTA balloon. During the procedure, the balloon allegedly ruptured at 17 atm while being used within a stent, below its rated burst pressure of 20 atm. There was no reported patient injury.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50914940 ↗MalfunctionSep 28, 2026Material Rupture
On (b)(6) 2026, a patient underwent a percutaneous transluminal angioplasty (PTA) procedure using a Conquest balloon catheter. During the procedure, the balloon was initially inflated to 16 atm and then deflated. It was subsequently advanced over the guidewire to the next lesion site. As the pressure was gradually increased to 25 atm, further inflation became impossible. The balloon was then deflated and retrieved, at which time a rupture was observed. The procedure was successfully completed using another balloon. There was no reported patient injury.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50913801 ↗MalfunctionSep 9, 2026Deflation Problem; Entrapment of Device
On (b)(6), 2026, a patient underwent an angioplasty procedure in left forearm using the Conquest PTA balloon. During the procedure, following balloon dilation, the device could not be withdrawn. To facilitate retrieval, the balloon was punctured with a cannula to achieve deflation, after which it was successfully removed from the patient. The procedure was completed using another balloon. There was no reported patient injury.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50687620 ↗MalfunctionSep 8, 2026Material Rupture
On (b)(6) 2026, a patient underwent a percutaneous transluminal angioplasty procedure using a Conquest balloon catheter. During the procedure, while using a Bard Ultra High Pressure Balloon, it was observed that the inflation device repeatedly lost pressure during inflation attempts. The balloon was subsequently removed, and a simulated inflation test was performed outside the body, revealing a small tear in the balloon. Upon inflation, fluid sprayed directly from the tear, resulting in insufficient pressure. The procedure was successfully completed after the balloon was replaced with another balloon. There was no reported patient injury.

Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50516918 ↗MalfunctionAug 24, 2026Material Rupture
On (b)(6) 2026, a patient underwent a percutaneous transluminal angioplasty procedure using a Conquest balloon catheter. During an intraoperative balloon dilation procedure for an arteriovenous fistula in the patients right forearm, the high pressure balloon ruptured before reaching its rated burst pressure. The balloon was immediately replaced, and the procedure was completed successfully. There was no reported patient injury.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50401317 ↗MalfunctionAug 13, 2026Difficult to Remove; Material Rupture
On (b)(6) 2026, a patient underwent a percutaneous transluminal angioplasty (PTA) procedure using the Conquest Balloon Catheter. During the procedure, the balloon catheter allegedly experienced a longitudinal rupture, and there was difficulty retracting the balloon through the introducer sheath following the rupture. The procedure was completed using another device. There was no reported patient injury.

MAUDE report 50231822 ↗MalfunctionAug 4, 2026Difficult to Remove; Material Rupture
On (b)(6)2026, a patient underwent a percutaneous transluminal angioplasty (PTA) procedure using a Conquest balloon catheter for the treatment of an autologous arteriovenous fistula in the left forearm. During the procedure, the balloon catheter was advanced over the guidewire to the target lesion and subsequently ruptured during deflation at 20 atm. Following the balloon rupture, difficulty was encountered while retracting the balloon catheter through the introducer sheath. As a result, the balloon catheter and introducer sheath were removed together as a single unit. The procedure was subsequently completed using another device. There was no reported patient injury.
Patient problems: No Clinical Signs, Symptoms or Conditions
MAUDE report 50127311 ↗MalfunctionJul 31, 2026Material Rupture
On (b)(6), 2026, a patient underwent an angioplasty procedure using a Conquest PTA balloon for the treatment of stenosis of an autologous arteriovenous fistula in the left upper limb. During the procedure, a pinhole rupture of the balloon was noted. The procedure was subsequently completed using an alternative balloon catheter. There was no reported patient injury.

A patient was prepared for a percutaneous transluminal angioplasty (PTA) procedure using a Conquest Balloon Catheter. Prior to the procedure, upon removal of the device from its packaging, a split was observed in the catheter, rendering the device unusable. A replacement balloon catheter was used to complete the procedure. There was no patient contact.
