We found 11 models of GlidePath by Bard Peripheral Vascular, Inc. in the FDA's device registry (GUDID). Together they are named in 41 adverse event reports in the FDA's MAUDE database, received between Oct 12, 2020 and Feb 3, 2026 — classified as 3 injuries, 38 malfunctions.
The most frequently reported specific device problem is “Deformation Due to Compressive Stress” (19 reports).
Reports come from manufacturers, hospitals and patients and are not verified by the FDA. A report does not prove the device caused the event. Widely used products collect more reports, so totals can't be used to compare products or estimate failure rates. Only reports that name a model's exact device identifier are counted. InjuryFeb 3, 2026Fluid/Blood Leak; Obstruction of Flow; Material Integrity Problem
ON (B)(6) 2026, A PATIENT UNDERWENT A DIALYSIS CATHETER PLACEMENT PROCEDURE USING GLIDEPATH. DURING THE PROCEDURE, IT WAS REPORTED THAT BLEEDING WAS OBSERVED FROM THE CUFF-SIDE EXIT PORT IMMEDIATELY UPON DEVICE CLOGGING. REPORTEDLY, A MACROSCOPIC INSPECTION DID NOT REVEAL ANY VISIBLE TEAR; HOWEVER, WHEN A PULL-AND-PUSH ACTION WAS APPLIED FROM THE BLUE SIDE, BLOOD WAS SEEN SEEPING FROM THE TUNNEL, INDICATING AN INTERNAL LEAK. FURTHERMORE, LEAKAGE COULD NOT BE REPRODUCED WITHOUT APPLYING CONSIDERABLE FORCE, MAKING IT DIFFICULT TO IDENTIFY THE PRECISE LOCATION OF THE DAMAGE. THERE WAS NO REPORTED PATIENT INJURY.
Patient problems: Extravasation; Hemorrhage/Blood Loss/Bleeding; No Clinical Signs, Symptoms or Conditions
MAUDE report 24243043 ↗MalfunctionJan 14, 2026Suction Problem; Malposition of Device
ON (B)(6) 2025, A PATIENT UNDERWENT FOR A DIALYSIS CATHETER PLACEMENT PROCEDURE USING GLIDEPATH. DURING A DIALYSIS CATHETER PLACEMENT PROCEDURE, THE CATHETER TIP WAS ALLEGEDLY POSITIONED SHALLOWER THAN INTENDED, RESULTING IN POOR BLOOD WITHDRAWAL. DUE TO INSUFFICIENT DEPTH AND INADEQUATE BLOOD RETURN, THE CATHETER WAS REMOVED AND REPLACED WITH A LONGER SIZE CATHETER TO ACHIEVE PROPER PLACEMENT. THERE WAS NO REPORTED PATIENT INJURY.

ON NOV 04, 2025, IT WAS REPORTED THAT AFTER REMOVING A 13F X 19CM GLIDEPATH CATHETER FROM A PATIENT, THE CUFF WAS FOUND TO BE DETACHED FROM THE CATHETER. THE CUFF WAS SUBSEQUENTLY RETRIEVED FROM THE SOFT TISSUE, AND IT WAS NOTED THAT BOTH THE CUFF AND A SLEEVE PORTION OF THE CATHETER HAD SEPARATED FROM THE ORIGINAL DEVICE. FURTHER PROBING AND DISSECTION WITH INSTRUMENTS AND ULTRASOUND GUIDANCE USED TO VISUALLY SEE AND REMOVE THE CUFF PORTION. THE CURRENT STATUS OF THE PATIENT IS UNKNOWN.

MAUDE report 23641577 ↗InjuryNov 14, 2025Fracture; Loose or Intermittent Connection; Air/Gas in Device; Fracture; Loose or Intermittent Connection; Air/Gas in Device
¿ON (B)(6), A PATIENT UNDERWENT A DIALYSIS CATHETER PLACEMENT PROCEDURE USING A GLIDEPATH HEMODIALYSIS CATHETER. ON (B)(6) 2025, FOLLOWING INITIATION OF DIALYSIS TREATMENT, THE PRESENCE OF AIR BUBBLES WAS OBSERVED IN THE HEMODIALYSIS CIRCUIT, SPECIFICALLY IN THE RED LINE CONNECTED TO THE BLUE LUMEN OF THE GLIDEPATH CATHETER. NO VISIBLE FISSURES WERE INITIALLY IDENTIFIED, AS THE SUSPECTED BREAK WAS LOCATED BENEATH THE PATIENT'S TEGADERM DRESSING. THE CATHETER WAS LATER FOUND TO BE RUPTURED AT THE JUNCTION BETWEEN THE OUTER CATHETER TUBING AND THE PLASTIC HUB. THE PATIENT WAS TRANSFERRED TO INTERVENTIONAL RADIOLOGY FOR AN EMERGENCY EVALUATION OF THE DIALYSIS LINE. AFTER THE DIALYSIS LINE WAS CHANGED AND THE DEFECTIVE CATHETER WAS RETRIEVED, THE CATHETER BREAK WAS CONFIRMED. THE CURRENT STATUS OF THE PATIENT WAS UNKNOWN.
